Please use these medical release forms to send your child’s medical records from your previous doctors. We have separate forms for Kennesaw and Marietta locations for your convenience.
What should I bring to my first visit?
To ensure a smooth first visit, please bring your child’s insurance card, immunization records, any relevant medical history, and a list of current medications.
If you are coming in for a newborn visit days after hospital discharge, we ask that you please bring hospital paperwork with you to your first visit.
You should receive a text or email from Phreesia with paperwork to fill out online prior to your visit.
Do you take my insurance?
We accept most commercial insurance plans as well as Medicaid, CareSource (except CareSource MarketPlace), and PeachState. We do not accept the following: Kaiser HMO and Added Choice POS, Amerigroup, Oscar, Ambetter, or CareSource Marketplace. Please note that when we schedule your first appointment, we will transfer you to our billing department to document your insurance and verify coverage.
I am transferring into East Cobb Pediatrics, what do I need to do?
To transfer to our practice you will need to have your previous practice send over your medical records. To do this, please fill out and send one of these forms to your office – depending on which location you’ll be transferring to.
