Please complete an ADHD packet and turn it in before scheduling a parent consult.

The follow-up Vanderbilt form will be provided through Phreesia. We ask that you complete it before Med Check visits.

Please download the form, select the location you are transferring to, and submit it to your previous pediatrician.

Please complete a Medical Record Release form to securely retrieve and transfer your records out of East Cobb Pediatrics.
