Transferring In to East Cobb Pediatrics

Thank you for choosing East Cobb Pediatrics! Before your first visit, we’ll need to get your child’s medical records from their previous physicians.

    Make sure you indicate which office you will be transferring to.

  • Send to your previous doctor

    Email, fax, or drop the form off at your previous doctor.

  • We’ll take care of the rest

    That’s it! Your doctor will securely deliver your child’s records directly to us.

Transferring Out of East Cobb Pediatrics

Fill out this form to retrieve your medical records and/or begin the process of transferring out of East Cobb Pediatrics.

There is a fee for all requests to transfer medical records out, $6.50 for electronic retrieval via secure email or $20.00 for a paper copy. The total fee must be paid before the emailing, picking up, or mailing out of records. Please allow 1 week for processing and sending of records.
