5 Days First office - extended visit. You will see our newborn specialist and lactation consultant. 
1 Month Hepatitis B #2 (GSK) Examination and Anticipatory Guidance
2 Months Pentacel (Sanofi)(Dtap, IPV,HIB), Prevnar (WY), Rotateq (MRK) Examination and Anticipatory Guidance
4 Months Pentacel (Sanofi), Prevnar (WY), Rotateq (MRK)  Examination and Anticipatory Guidance
6 Months Pentacel (Sanofi), Prevnar (WY), Rotateq (MRK)   Examination and Anticipatory Guidance
9 Months Hepatitis B #3 (GSK)  Examination and Anticipatory Guidance. CBC (Complete Blood Count) - Finger Stick
12 Months Varivax (MRK) #1 vaccine against Chicken Pox, Hepatitis A (GSK) #1 if in daycare and born after January 1, 2006. 
Examination and Anticipatory Guidance. 
15 Months MMR (MRK), Prevnar (WY)  Examination and Anticipatory Guidance. 
18 Months Pentacel (Sanofi), Hepatitis A #2 (if received at one year visit.) 
2 Years Comprehensive toddler exam and discussion of toddler hood issues.  Hepatitis A if not received yet. The second dose is due 6 months later.   CBC (Complete Blood Count) - Finger Stick
3 Years Examination and Anticipatory Guidance                        
Dental Examination recommended. 
4 Years  Daptacel (Sanofi), IPV (Polio) (Sanofi) Examination and school readiness discussion. Hearing and Vision Exam will be performed at this visit or at the 5 year visit. 
5 Years MMR (MRK)#2, Varivax (MRK) #2 Examination and Anticipatory Guidance. Hearing and Vision exam if not completed at 4 year visit. 
6-10 Years Yearly examinations recommended                         CBC at 6 years
11 years TDAP - Vaccine against Tetanus and Pertussis
Menactra - Vaccine against Meningococcal disease.            Examination and Anticipatory Guidance
12 to 14 years Yearly examinations recommended                        
CBC at 12 years and then yearly on female patients.  Gardasil - Vaccine against Human Papillomavirus/Cervical Cancer recommended for all patients.
This is a threee dose vaccine.
15 years Examination and Anticipatory Guidance  Catch up any missed vaccines
16 to 20 years Examination and Anticipatory Guidance. Catch up any missed vaccines
  We recommend a flue vaccine to all patients 6 months and older yearly.
