Comprehensive toddler exam and discussion of toddler hood issues.
Hepatitis A if not received yet. The second dose is due six months later.

Dental Examination recommended

4-5 Years

Examination and school readiness discussion
DTaP # 5, Polio # 4, and Proquad # 2

6-10 Years

Yearly Examinations recommended
CBC at 6 years 
 

11 Years

Examination and Anticipatory Guidance
TDaP-Vaccine against Tetanus and Pertussis
Menactra-Vaccine against Meningococcal disease

12-14 Years

Yearly Examinations recommended
CBC at 12 years and then yearly on female patients
Gardasil-Vaccine against Human Papillomavirus/Cervical
Cancer recommended for all female patients 14 years and up.  This is a three dose vaccine


15 Years

16-20 Years

Examination and Anticipatory Guidance
Catch up any missed vaccines
  We recommend a flu vaccine to all patients 6 months and older yearly.
